摘 要: 目的 了解基层医务人员职业紧张现状,构建职业紧张风险预测模型列线图。方法 采用工作内容量表(Job Demand-Control Questionnaire,JCQ)、付出-回报失衡量表(Effort-Reward Imbalance Questionnaire,ERI),于2021年8月整群抽取河北省雄安新区三县7家公立医院2 325名医务人员进行职业紧张调查;应用Logistic回归筛选职业紧张危险因素,构建列线图模型。结果 工作要求-自主失衡(Job Demand-Control,JDC)型和ERI型职业紧张得分[M(Q)]分别为0.99(0.28)分和1.02(0.31)分,发生率分别为45.59%和54.54%。Logistic回归分析显示,医院类型、科室、职称、工作班制和平均日工作时长是JDC型职业紧张的影响因素,医院类型、科室、职称、工龄、用工性质、工作班制和平均日工作时长是ERI型职业紧张的影响因素(P<0.05)。JDC、ERI型职业紧张列线图预测模型的一致性系数分别为0.613和0.700,校准曲线与理想曲线基本一致,有良好的区分度和准确性。结论 雄安新区基层三县医务人员职业紧张水平较高,本文构建的列线图模型有较好的预测能力。 |

Abstract: Objective To investigate the current situation of occupational stress of primary medical staff,and construct risk predictive nomogram model for predicting the occupational stress. Methods Using the Job Demand-Control Questionnaire(JCQ)and the Effort-Reward Imbalance Questionnaire(ERI),a cluster sampling of 2 325 medical staff from 7 public hospitals in three counties of Xiong'an New Area was conducted in August 2021 to investigate the occupational stress,the multivariate Logistic regression was used to screen the risk factors of occupational stress,then,the risk predictive nomogram models for two types of occupational stress were constructed. Results The results showed that the occupational stress scores of Job Demand Control(JDC)type and ERI type were 0.99(0.28)and 1.02(0.31)respectively,with incidence rates of 45.59% and 54.54%. The results of multiple Logistic regression analysis showed that the hospital type,department,professional title,work shift system and average daily working hours were the influencing factors of JDC type occupational stress;while the hospital type,department,professional title,length of service,employment nature,work shift system and average daily working hours were the influencing factors of ERI type occupational stress(P<0.05). The consistency coefficients of JDC type and ERI type constructed nomogram models were 0.613 and 0.700,respectively,and the calibration curves were basically consistent with the ideal curve,with good discrimination and accuracy. Conclusion The results suggested that the occupational stress level of grass-roots medical personnel in three counties of Xiong'an New Area was relatively high,the major type of occupational stress was ERI,and the column chart prediction model constructed in this paper had good prediction ability. |
