老年煤工尘肺并发肺癌的危险因素分析

摘 要: 目的:探究老年煤工尘肺(CWP)并发肺癌的危险因素。方法:收集我院2018年6月至2021年6月期间收治的老年煤工尘肺(CWP)患者临床资料,按照是否并发肺癌分为并发肺癌组和未并发肺癌组,分析老年CWP并发肺癌的危险因素。结果:单因素、多因素logistic回归分析结果显示年龄≥75岁、吸烟、接触粉尘工龄>15年、每日接触粉尘时间>8 h、CWP分期Ⅲ期是老年CWP并发肺癌的危险因素,基于logistic回归预测老年CWP并发肺癌的AUC为0.736,敏感度、特异度分别为79.46%、61.88%。结论:对老年CWP并发肺癌的危险因素加以防控可以有效降低肺癌并发率。

Abstract: Objective: To explore the risk factors of elderly coal workers’ pneumoconiosis (CWP) complicated with lung cancer. Methods: The clinical data of elderly patients with CWP admitted to the hospital from June 2018 to June 2021 was collected. The patients were divided into lung cancer group and non-lung cancer group. The risk factors for lung cancer in elderly patients with CWP were analyzed. Results: Univariate analysis and multivariate logistic regression analysis results showed that age ≥ 75, smoking, working years with dust exposure > 15 years, daily exposure to dust time > 8 h, and stage III CWP were risk factors for lung cancer in elderly patients with CWP.The AUC of predicting elderly CWP complicated with lung cancer based on logistic regression was 0.736, with a sensitivity and specificity of 79.46% and 61.88%, respectively. Conclusion: Prevention and control of the risk factors for lung cancer in elderly patients with CWP can effectively reduce the incidence of lung cancer.
