摘 要: 目的 研究矽肺患者血清中白介素17(IL-17)、T淋巴细胞亚群的表达水平,并探讨吸烟对患者上述指标的影响。方法 选取60例矽肺患者作为矽肺组,60名无粉尘接触者作为对照组。分析矽肺组不同诊断期别、血氧分压、肺通气功能、小气道功能是否正常及有无吸烟患者间IL-17、T淋巴细胞亚群水平差异。结果 与对照组比较,矽肺组IL-17表达水平升高,CD3+ T细胞、CD3+CD4+ T细胞水平降低;矽肺叁期患者CD3+ T细胞低于矽肺壹期(P<0.05)。矽肺患者肺通气功能、血氧分压与其IL-17、T淋巴细胞亚群水平无明显相关(P>0.05)。吸烟的矽肺患者CD3+ T细胞水平低于已戒烟者,CD3+CD8+ T细胞水平低于不吸烟者及已戒烟者,CD3+CD4+/CD3+CD8+ T细胞比值高于不吸烟者及已戒烟者(P<0.05)。矽肺壹期吸烟患者CD3+ T、CD3+CD8+ T细胞水平低于已戒烟者;贰期吸烟患者IL-17表达水平高于不吸烟者及已戒烟者;叁期吸烟者CD3+ T细胞水平低于不吸烟者,且CD3+CD4+/CD3+CD8+ T细胞比值升高(P<0.05)。结论 IL-17是参与矽肺肺间质炎症的细胞因子,吸烟会增加矽肺患者IL-17的表达,影响T淋巴细胞水平,存在诱发患者肺间质炎症及抑制细胞免疫功能的可能。 |

Abstract: Objective To study the changes of interleukin-17(IL-17)and T-lymphocyte subsets in blood of patients with silicosis,and to explore the effect of smoking on the changes. Methods 60 silicosis patients were selected as silicosis group,and 60 people without dust exposure served as control group. Detect the serum levels of IL-17 and T-lymphocyte,analyze their differences among different groups and different smoking habit;meanwhile,the results of blood gas analysis,and lung ventilation function were analyzed as well in silicosis group. Results The results showed that compared with control group,the levels of IL-17 in silicosis group were increased,the levels of CD3+ T cell and CD4+ T cell were decreased,and the CD3+ T cell was lower in stage III silicosis subgroup compared with that of stage I silicosis subgroup(P<0.05). There was no significant correlation between the blood levels of IL-17 and T-lymphocyte subsets and pulmonary ventilation function or blood oxygen partial pressure,in silicosis patients(P>0.05). Furthermore,the level of CD3+ T cell in smoking subgroup of silicosis patients was lower than that of the ex-smoking subgroup,and the level of CD3+CD8+ T cells was lower than those non-smoking subgroup and ex-smoking subgroup,but CD3+CD4+ T/CD3+CD8+ T level was higher than that of non-smoking subgroup and ex-smoking subgroup(P<0.05);additionally,the blood levels of CD3+ T and CD3+CD8+ T cells in smoking group was lower than that of ex-smoking subgroup in stage I silicosis patients,the blood level of IL-17 in smoking subgroup was higher than that of non-smoking group and ex-smoking subgroup in stage II silicosis patients,and in stage III silicosis patients,the blood level of CD3+ T cells of smoking subgroup was lower than that of non-smoking group,but CD3+CD4+ T/CD3+CD8+ T was higher than that of non-smoking group(P<0.05). Conclusion The results suggested that IL-17 might be one of the cytokines involved in lung interstitial inflammation in silicosis patients,smoking could increase the expression of IL-17 in blood of silicosis patients,thereby affect the blood level of T-lymphocyte,and then give the possibility of inducing pulmonary interstitial inflammation and inhibiting cellular immune function. |
