摘 要: 目的 探究手传振动(HTV)作业工人指端温度觉阈值(TPT)水平及其影响因素,为早期诊断手臂振动病(HAVD)提供依据。方法 对某运动器材公司187名工人开展横断面研究,其中接振组69人、对照组118人。采用HVLab温度觉测试仪进行测量,多元线性回归分析指端TPT的影响因素。结果 接振组工人主力手食指与小指的热觉阈值水平[(46.4±5.7)℃、(45.8±5.6)℃]均高于对照组[(43.4±4.7)℃、(43.7±4.2)℃](P<0.05),且中性区范围[(22.8±8.9)℃、(24.7±9.3)℃]均大于对照组[(18.3±7.6)℃、(21.2±7.8)℃](P<0.05);接振组主力手食指冷觉阈值[M(P25,P75)]24.3(21.0,27.5)℃低于对照组[26.2(22.7,28.3)℃](P<0.05)。多元线性回归分析显示,年龄、身高和手指麻木为主力手食指热觉阈值和中性区的影响因素(P<0.05),手指麻木为主力手冷觉阈值的影响因素(P<0.05)。年龄对主力手小指温度觉阈值和中性区具有正向预测作用(P<0.05)。结论 长期暴露于HTV会导致接振工人指端TPT变化,且接振工龄、年龄和手指麻木是TPT的影响因素。 |

Analysis on influencing factors of fingertip thermotactile perception threshold of hand-transmitted vibration operators |

Abstract: Objective To measure explore the fingertip thermotactile perception threshold(TPT)of workers exposed to hand-transmitted vibration(HTV)and its influencing factors,thereby provide some evidences for the early diagnosis of hand-arm vibration disease(HAVD). Methods A cross-sectional study was conducted on 187 workers from a sports equipment company(69 in vibration exposed group and 118 in control group). HVLab temperature sensor was used for measuring,and multiple linear regression was used to analyze the influencing factors of TPT on the fingertip. Results The results showed that the thermal perception thresholds of the index finger and the end finger of the main hand in vibration exposed group[(46.4±5.7)℃ and(45.8±5.6)℃]were all higher than those of control group[(43.4±4.7)℃ and(43.7±4.2)℃],respectively(P<0.05),and the neutral zone ranges[(22.8±8.9)℃ and(24.7±9.3)℃]were also higher than that of the control group[(18.3±7.6)℃ and(21.2±7.8)℃],respectively(P<0.05). The cold sensation threshold of the index finger[M(P25,P75)]of the main hand in vibration exposed group was 24.3(21.0,27.5)℃,lower than that of control group[26.2(22.7,28.3)℃](P<0.05). Multiple linear regression analysis showed that age,height and finger numbness were the influencing factors for thermal perception threshold and neutral zone of the index finger of the dominant hand(P<0.05);finger numbness was the influencing factor of the cold sensation threshold(P<0.05);and age had a positive predictive effect on TPT and neutral zone of the main hand terminal finger(P<0.05). Conclusion The results suggested that long-term exposure to HTV could lead to the difference of fingertip TPT,and the exposed time of HTV,age,finger numbness could affect TPT. |
