摘 要: 比较尿汞mg/L、μg/gCr两种表达方式应用于汞中毒诊断及预防实际中的效果。 方法 收集188名正常人(对照组)、1046 名职业接触汞人员(试验组)随机尿样于聚乙烯塑料瓶中,用校正后的F732 Ⅴ型测汞仪、酸性氯化亚锡还原 冷原子荧光法测定尿总汞含量,用手工碱性苦味酸法检测尿液肌酐含量,尿汞结果分别以mg/L、μg/gCr两种方式报告。 结果 对照组及试验组尿总汞含量两种表达结果均成正偏态分布,对照组尿汞mg/L的几何均数为0.00123、尿汞μg/gCr的几何均数为0.96,两种报告方式的结果之间存在正相关关系( r =0.9967, P<0.01);试验组尿汞mg/L的几何均数为0.00287、尿汞μg/gCr的几何均数为2.42,两种报告方式的结果之间存在正相关关系( r =0.9935, P<0.01)。以0.01 mg/L与4 μg/gCr为正常人参考值标准判断试验组检测结果时,差异有统计学意义(P<0.01)。[ 结论 以0.01 mg/L为正常人参考值做标准判断结果的漏诊率较高;判断个体样本的测定值是否超过正常参考值或生物限值,以μg/gCr方式报告尿汞含量更符合临床实际;但大样本调查研究,以mg/L、μg/gCr两种方式报告得出的结论差异无统计学意义 |

Comparison of two expression ways of urinary mercury in practical application for diagnosis and prevention of mercury poisoning |

Abstract: To compare the features of two expression ways(mg/L and μg/gCr)for urinary mercury commonly applied in mercury poisoning diagnosis and prevention. Methods 188 non mercury exposure persons and 1046 mercury exposed workers were sellected as control group and exposed group.The random urines smaples were collected in PVC bottles,detected by acidic stannous chloride reduction cold atomic fluorescence spectrometry with a F732 V mercury,urinary creatinine was manually detected by alkaline picrate metho,and results of urine mercury were expressed as mg/L or μg/gCr,respectively Results The results showed that two expressions of result were positively skewed distribution,the geometric mean of urinary mercury for control group were 0.00123 mg/L and 0.96 μg/gCr respectively,there was well positively correlated among them( r =0.9967, P< 0.001);while the geometric mean of urinary mercury in exposed group were 0.00287 mg/L and 2.42 μg/gCr respectively ,there was also a positively correlation among them(r=0.9935 P<0.01;and based on 0.01 mg/L and .4 μg/g Cr as normal reference to judge the results of exposed group respectively, there was significant difference statistically(P<0.01). Conclusion If using 0.01 mg/L as normal reference value to judge exposed workers,the missed diagnosis rate was higher than that of 4 μg/gCr ,suggesting that μg/gCr it is more suitable for clinical practice,but there seems no significant differences between mg/L or μg/gCr large samples groups comparison |
